Patient notices: privacy policy for the Sky Eye Clinic website

These patient notices summarize the Sky Eye Clinic privacy policy: what personal information the clinic's website collects, how it is used and kept, and how you can view, correct or withdraw it. The source policy took effect in 2010 and was last modified on April 3, 2012.

What is collected and why

This privacy policy covers the Sky Eye Clinic website. Sky Eye Clinic collects personal information for membership registration, consultation and service requests. The policy lists these items: name, date of birth, gender, resident registration number, login ID, password, phone numbers, address, email, occupation, service usage records, access logs, cookies and IP information. It is collected through the website and various postings.

The information is used to fulfil contracts and provide services, to manage members (identity checks, preventing misuse, handling complaints, sending notices), and for marketing, such as developing services, sending event information and analyzing site usage.

How long it is kept and how it is destroyed

Under the privacy policy, information is destroyed without delay once its purpose is achieved. One item is kept: the login ID, permanently, so the same ID cannot be registered again after withdrawal.

  • Electronic files are deleted by methods that cannot reproduce the records.
  • Paper records are shredded or incinerated.
  • Before destruction, data is stored separately for a set period under internal policy and other laws, and is not used for other purposes unless the law requires it.

Your rights and cookies

The privacy policy lets you view or change your registered information at any time, or cancel your membership through the website after identity verification. You can also contact the personal information manager in writing, by phone or by email. If you request a correction, the clinic stops using the information until it is fixed and tells any third party that received incorrect data.

The website uses cookies to analyze visits and understand your interests. You can allow all cookies, be asked each time, or block all cookies in your browser settings. Blocking them may make some services harder to use.

For children under 14, the clinic obtains consent from a legal representative, who may also view, correct or withdraw the child's information.
